Please see the attached letter and forms from Melissa Ajao at Ludeman regarding the Illinois State ID DMV Event for Ludeman Residents on May 13th.

If you are interested in having your loved one get an Illinois State Identification Card, please fill out the attached forms and return them to Melissa Ajao at Ludeman.

Melissa also said the letter and forms have been mailed to guardians.
