We have been made aware that you, as a Legal Guardian should file regular reports on your individual and that there is a requirement in the Illinois Probate Act that Legal Guardianship holders are required to successfully complete the Illinois Legal Guardianship Training module to maintain your Legal Guardianship of your developmentally disabled person.
Below you will find information on these two recommendations.

Illinois Guardianship Training
When you have successfully completed the Legal Guardianship training, you need to keep a copy of your training certificate. You also need to file your training certificate in the County where you obtained your Legal Guardianship.
We also recommend a copy of the certificate be provided to the Ludeman Center Records Department.
The Illinois Guardianship Training slide 33 also suggests the following Residential Placement rights for your individual to be included in your Legal Guardianship paperwork:
YOUR REQUIRED GUARDIANSHIP REPORT
In Illinois, most Circuit Courts require you to file a report on the status of your individual for whom you are their Legal Guardian. Listed below are links based on what County you obtained your Legal Guardianship. You should fill out the report, maintain a copy for your own files, and return the report to the specific County Court where you received your Legal Guardianship.
