PFLC – IMPORTANT – Suburban Access @ Ludeman 2/27 & 2/28

Hello Parents and Friends of Ludeman Center,

To meet the Americans with Disabilities Act and Olmstead requirements, agencies that offer services and programs to the Developmentally Disabled community interview individuals that are interested in various types of services and placement. 

Suburban Access is an agency located in Homewood, Illinois.  The agency will be visiting Ludeman on Monday and Tuesday to interview Ludeman residents if they are interested in services that are offered in the community to the Developmentally Disabled population.

Contact your Ludeman QIDP (Qualified Intellectual Disabilities Professional) that is assigned to your loved one’s care to let them know if you want or do not want your loved one interviewed.

I have attached a copy of a letter many of us that are parents, siblings, and legal guardians for a loved one that lives at Ludeman need to be aware of.  Interviews will be conducted this Monday and Tuesday with the residents.  NOTE: The letter states that they will need permission from you to conduct this interview.

If you do not know who the QIDP is assigned to your loved one’s care, call the Ludeman Switchboard at 708-283-3000 and you will be directed to someone that supports your loved one’s home.
