I hope the New Year has brought you renewed hope that the pandemic will come to and end soon. We would like to share some news, and a request to make sure that if you are a Parent/Guardian you have provided the Ludeman Center your consent for the Moderna Vaccine that is now available to the ELDC residents.

GREAT NEWS!! THE MODERNA VACCINE HAS ARRIVED AT THE ELISABETH LUDEMAN CENTER!!
If you have not received your consent form to administer the vaccine to your loved one that resides at Ludeman, click on the links below for the fact sheet and Moderna Vaccine consent forms. Since the Moderna Vaccine requires two doses of the vaccine at different intervals to be effective, you must fill out and send in two forms. One for the first dose, and another for the second dose. Please make this a top priority so your loved one can receive the vaccine.
